diving officers record cards

Hi All
Does anyone out there have a blank copy of the older style diving officers record sheet ( the ones for blue Qual book)? That they could possibly scan and send me BSAC mail shop no longer do them . I'm just in the process of giving the club training records an overhaul. Thankyou:)

I can't help you out Boomer, but watch out as there are at least 3 different syllabuses (bi?) recorded in various editions of the blue log book.
