Do any clubs use medical rebreathers like the one in this link (http://www.teigndivingcentre.co.uk/content/view/82/10/)?
No, but I have put someone on O2 from my rebreather.

Not sure, but I recall reading the full write-up in DAN's "Alert Diver" some time last year.
Teignmouth Dive Centre were very nice people and were competitive ly pricing their chandlery when I was there a few years ago. Since the link is their's, perhaps ask them?

I think it is a DAN product. I remember them demonstrating it in 2006.

I seem to remember that DAN launched something called the REMO which was a forerunner to this about 10 years ago at DEMA.
I am aware that a number of divers have been successfully treated on the surface on a rebreather.
I think DAN might well be exhibiting at LIDS, so give this would be an ideal opportunity for you to talk to them there, or indeed as Mike said, contact the linked dive centre direct.
