Can anyone tell me who signs off a qualification book if you DO isn't an instructor?

The DO, on the recommendation of the NQI that carried out or supervised the training.

From the BOH

A Branch Diving Officer should hold at least the qualification of BSAC Advanced Diver and ideally be a BSAC Nationally Qualified Instructor. If not, they will require a Nationally Qualified Instructor to supervise Branch instruction and assessments, and recommend candidates before they are able to award a diving qualification.

So you just need your Instructor to reccommend you to the
DO and he/she can then sign you up.

4.4.4 Authority to Award BSAC Diving Qualifications
At Branch level, the authority to award diving qualifications (Section 4.1.1) has been delegated (Section 3.2.3) solely to the Branch Diving Officer (Section 3.3.1). For Dual Members (Section 3.4.5) the authority lies with the Branch Diving Officer of their primary Branch. A Nationally Qualified Instructor (Section 4.1.1) must recommend prospective candidates for the award of a diving qualification to the Branch Diving Officer. This applies to both snorkelling and SCUBA qualifications. Branch Committee approval is not required because:

* The National Diving Officer has delegated the authority to the Branch Diving Officer, not to the Branch Committee.
* The National Diving Officer is not prepared to accept a situation where Committee Members less qualified could veto the decision of a Branch Diving Officer and the Nationally Qualified Instructor recommending a member for a diving qualification.

Qualifications above the level held by the Branch Diving Officer:
In the event of wishing to qualify an Advanced Diver when neither the Nationally Qualified Instructor and/or Branch Diving Officer hold this qualification themselves, the Branch must seek guidance, ideally from the BSAC Regional Coach, more highly qualified Nationally Qualified Instructors from within the Branch or from neighbouring Branches. Under these circumstances, the more highly qualified Nationally Qualified Instructor or Coach should make the decision as to whether the qualification should be awarded and if so, should either:

* Recommend to the Branch Committee that the Branch Diving Officer awards and signs the qualification.
* Confirm the award and sign the qualification himself.

In either case, the course of action taken must be recorded in Branch Committee Minutes (Section 3.3.4) and subsequently entered in Branch Training Records.
