Can anyone help me with locating a doctor or surgery that can sighn my pay and die instantly form. My local doc recomended a specalist doctor for such an examination as they understand the pressures that the body will be under.
i'm in the Cheltenham area so if any of you friendly BSAC folk can help i'd be very happy.

See http://www.bsac.org/page/42/medical-matters.htm where you will find the Self Certification Medical Form and if you need to refer to a Dr the UKSDMC list can also be found from the page.

If you're doing the "Pay And Die Immediately" course then the form Edward told you about is null and void - PADI have their own and will not accept any other.

Get a copy, and if there is a answer "yes" to any of the questions then you must see a doc, not necessarily a Diving Doc, PM me with what your problem is and I'll let you know. If its something that requires a more in depth look then thats where the list at the top of this thread

the uksdmc list of medical referees

Comes in - find your local guy and book yourself an appointment.

Dont fill it online BTW, just print off and fill it in.

Scroll down A-6 is the Doc one.

TBH though I'm a bit confused why you are doing the runaround. Any decent
dive outfit would send this to you on request.
